Colleen Ellis, M.D.

Dr. Ellis is Board Certified in Family Practice. She is a Florida native. She graduated Summa Cum Laude from the University of Tampa in 1984 and the University of Florida College of Medicine in 1988.

After receiving her M.D. she completed the Family Practice Residency Program at Hunterdon Medical Center in Flemington, NJ. She served as chief resident from 1989 to 1991 and was awarded the 1991 Senior Resident Award given by the New Jersey Academy of Family Physicians.

Dr. Colleen Ellis is Board Certified in Family Practice and is a Diplomat of the National Board of Medical Examiners and a Fellow of the American Academy of Family Physicians. She is NCQA Certified in Diabetes and Heart and Stroke Care. Josephine Polch, ARNP earned her Master of Science in Nursing in May, 2009 at the University of South Florida, Tampa, Florida.

JSA accepts most major health plans including Humana Gold Plus, Medicare, Tricare, PPOs and HMOs, and private pay patients.
